Our GISU Nurse, Kristie Barber, has arranged for a flu vaccine clinic at Folsom on Tuesday, October 15th. If you want your child to participate in this flu vaccination clinic, you must complete & return the required paperwork to our Health Office by Wednesday, October 9th.
No late or "day of" paperwork will be accepted.
If you need paper copies rather than electronic ones, just email Cody Fiala, our Health Assistant at cfiala@gisu.org, and she will put them in your child's backpack.
